血糖値計IC

説明

血糖値計は、血液サンプル中のグルコース濃度を検出する携帯式の測定器です。これらの機器は、主として糖尿病患者によって使用されます。

現在市販されている血糖値計には、連続式と個別(単一検査)式があります。さらに、連続式の血糖値計は医師の処方箋を必要とします。これらの血糖値計は、皮下に埋め込んだ電気化学センサーを使用して、設定された時間間隔で測定を行います。

単一検査式の血糖値計は、電気化学方式または光反射率方式を使用して、グルコース濃度(単位:mg/dLまたはmmol/L)を測定します。血糖値計の大部分は電気化学方式です。電気化学方式の検査ストリップは、高精度のバイアス電圧が印加される電極を備えています。印加された電圧によって検査ストリップ上に電気化学反応が発生し、その結果流れる電流は血液中のグルコース量に比例します。次にその電流がグルコース濃度に変換されて表示されます。

検査ストリップの各パケットには較正コードが含まれており、使用前にそのコードを較正のためにメーターに入力する必要がありますが、新しい検査ストリップ設計の中には較正のステップを無くしたものもあります。

血糖値計は、電気化学センサーと、ディスプレイを備えたバッテリ駆動のエンベデッドシステムで構成される携帯式の測定器です。エンベデッドシステムは、センサーの読み取り、センサー読み値の処理、その表示、およびオプションでその保存を行います。

これらの血糖値計は、サンプルのグルコース濃度を判定するために非常に高精度のアナログ処理を必要とします。光反射率測定式と電気化学式の血糖値計は、どちらも数ナノアンペアの範囲の電流に対応する分解能が必要です。これらの機器は一般に狭い温度範囲内で高精度を発揮するため、高精度の温度測定も必要です。

新しい設計の多くは読み値を保存し、記録のためにPCまたはスマートフォンに読み値を送信することができます。

電気化学式検査ストリップの構成


大部分の検査ストリップは、血糖値計メーカーごとにさまざまな独自仕様で作られています。それぞれ試薬の処方、電極数、チャネル数、試薬のバイアス方法などが異なります。最も単純な構成は、自己バイアス式検査ストリップ(図1)です。この検査ストリップには2つの電極があり、作業電極と接地されたコモン電極で電流を測定します。

1つの検査ストリップに複数のチャネルがある場合もあります。追加のチャネルは、リファレンス測定や初回の血液検出に使用したり、反応部位を血液で飽和させたりするために使用します。もう1つの構成は、両方の電極をアクティブに駆動し、コモン電極で測定を行うものです。さらに高度な別の設計に、カウンタ構成があります(図2)。

この構成では3つの電極があり、作業電極で電流を測定し、フォース/センス回路によってコモン電極と基準電極を駆動します。この構成は、測定の間、検査ストリップ上の反応部位のバイアス電圧がより正確に設定され、維持されるという重要な長所があります。この設計の短所としては、複雑さが増大すること、また、通電中のバイアス電圧を維持するため、より大きなヘッドルームを確保してフォース/センスアンプで負の振幅を可能にする必要があることが挙げられます。

センサーアナログシグナルチェーン


光反射率測定式と電気化学式の血糖値計は、両方とも数ナノアンペア(1桁ナノアンペア)の範囲の電流を分解する必要があります。血糖値計の許容誤差を満足するには、製造時における血糖値計の較正後、電源電圧、温度、および時間の変化に対して各部品の漏れやドリフトが極めて小さいことが必要です。容量性の電気化学式検査ストリップに接続するオペアンプで重要となる仕様は、超低入力バイアス電流(< 1nA)、高い直線性、および安定性です。オペアンプは、通常いずれの方式の血糖値計に対してもTIAとして構成します。電圧リファレンスで重要となる仕様は、温度係数が50ppm/℃以下、経時ドリフトが小さい、優れたラインレギュレーションとロードレギュレーションなどです。電気化学式検査ストリップのバイアス電圧の設定、および光反射率測定式検査ストリップのLED電流の設定には、10ビット/12ビットDACを使用します。電気化学式検査ストリップでは、検査ストリップに血液が塗布された時点で検出するために、コンパレータを使用する場合もあります。これによって、検査ストリップに血液が塗布されるまでの消費電力を節約するとともに、反応部位を血液で十分に飽和させることができます。ADCの要件は血糖値計の種類によって異なりますが、ほとんどの場合、検査結果の再現性を確保するには、14ビット以上の分解能と低ノイズが要求されます。ダイナミックレンジを拡大するためにADCの前に設定可能な利得段がある場合は、12ビットの分解能も使用されます。

温度測定


検査ストリップ上の血液の温度を測定するのが理想的ですが、通常は検査ストリップ近接の周囲温度を測定します。温度測定の精度は検査ストリップの種類や化学的性質によってさまざまですが、±1℃~±2℃が標準的な範囲です。この測定は、スタンドアロンの温度センサーICを使用するか、リモートサーミスタまたはPNジャンクションとADCの組み合わせによって行うことができます。ADCと同じリファレンスで駆動されるハーフブリッジ構成でサーミスタを使用すると、電圧リファレンスの誤差が解消されるため、より正確な結果が得られます。リモートまたは内部PNジャンクションは、高精度の高集積アナログフロントエンド(AFE)で測定することができます。

ユーザーインタフェース:ディスプレイ、音声


大部分の血糖値計は、マイクロコントローラに内蔵されたLCDドライバで駆動可能な約100セグメントの単純な液晶ディスプレイ(LCD)を使用しています。カラーディスプレイは、両方のセグメントLCDより高い追加の電圧を必要とします。1つまたは2つの白色LEDを使用することによってバックライトを追加することが可能です。

血糖値計の可聴式インジケータには、単純なブザーから、視覚障害に対応したより高度な音声機能まで、さまざまなものがあります。単純なブザーは、パルス幅変調(PWM)機能によって1つまたは2つのマイクロコントローラポートピンで駆動することができます。より高度な音声インジケータや検査結果記録用の音声録音機能も、オーディオコーデックをスピーカアンプやマイクロフォンアンプとともに追加することで実現することができます。

電源およびバッテリマネージメント


単純なディスプレイを備えた血糖値計は、リチウムコイン電池1個、またはAAA (単4形)のアルカリ1次電池2本で直接動作させることができます。電池の寿命を最大限に延長するために、この血糖値計では、3.6Vから2.2Vまで(リチウムコイン電池の場合)または1.8Vまで(アルカリ単4電池の場合)動作可能な電子回路が必要です。電子回路がより高い電源電圧やレギュレートされた電源電圧を必要とする場合は、ステップアップスイッチングレギュレータを使用することができます。スリープ回路がより低いバッテリ電圧で動作可能である限り、スリープモード時にスイッチングレギュレータの電源を切断し、バッテリで直接動作させることで、バッテリの寿命は延長されます。バックライト付きディスプレイやより高度なディスプレイを追加するには、より高い電圧や、場合によっては別の電圧も必要になります。この時点で、より高度なパワーマネージメント方式が必要になる可能性があります。バッテリチャージャと残量ゲージ回路を追加して、単一セルリチウムイオン(Li+)バッテリなどの再充電可能なバッテリを使用することができます。血糖値計でUSBが利用可能であれば、USBによる充電がユーザーにとって便利なオプションとなります。
