To Place a Prototype Order:

  • In the U.S. and Canada:
    • Online: Order On-Line from Maxim Direct. When the system asks you to indicate "Stage of Production," choose "PROTOTYPE."
    • By phone: Call 888-629-4642 (888-MAXIM-IC) or 972-371-6824 and ask for the "Prototype Order Desk." Credit and credit card orders are accepted.
    • By email: Download the Excel spreadsheet Order Form, fill out form, and indicate PROTOTYPE ORDER on the form. Email the completed form to: .

  • Worldwide:
